Pay Range: $33 - $59/hour. Employee benefits include, but are not limited to, health insurance (medical, dental, vision), 401(k) plan, and paid sick leave (depending on work location).

Key Responsibilities:

• Conduct initial in-person home visits to assess patients enrolled in the Virtual Care @ Home program.
• Develop and coordinate individualized care plans, treatment goals, and ancillary needs during home visits.
• Identify and enroll eligible patients from inpatient, ER, or clinic settings into the program, collaborating with Virtual Care physicians for plan approval.
• Educate patients and families on program goals, self-care, medication management, and disease-related information.
• Assess social barriers that could affect patient outcomes and work to address them to reduce readmissions or unnecessary hospital visits.
• Collaborate with care team members including lead coordinators, paramedics, social workers, and inpatient teams to ensure smooth transitions and coordinated care.
• Support continuous improvement of admissions and enrollment processes by working with management and leadership.
• Perform documentation and follow hospital policies and standards.

Key Requirements and Technology Experience:

• Skills-Registered Nurse, Telehealth or Homehealth
• Education: Associate’s, diploma, or bachelor’s degree in nursing (master’s preferred).
• Licensure: Active Registered Nurse (RN) license (single state or multi-state compact).
• Certifications: Current Basic Life Support (BLS) and Advanced Cardiac Life Support (ACLS) certifications (or equivalent).
• Experience: Minimum 3 years of acute care nursing in ICU or med-surg required, or minimum 5 years of home health or hospice preferred.
• Skills: Strong assessment, care planning, patient education, team collaboration, and communication skills.
