Role Overview:

We are seeking a compassionate and skilled Family Nurse Practitioner (FNP) to join our healthcare team in Charleston, WV. This is a hybrid position offering the flexibility of both in-person and remote work. The ideal candidate will deliver comprehensive primary care services, manage chronic conditions, and promote overall patient wellness through a holistic approach.

Key Responsibilities:

• Provide primary and preventive care services to patients of all ages.
• Conduct physical examinations, diagnose conditions, and manage treatment plans.
• Offer counseling and coordinate care for patients with chronic conditions.
• Participate in weight management programs and sexual health services.
• Administer hormone replacement therapy (HRT) and perform Botox treatments.
• Promote a holistic approach to patient health and well-being.
• Maintain accurate and timely patient documentation.
• Collaborate with multidisciplinary teams to ensure quality patient outcomes.

Qualifications:

• Active Family Nurse Practitioner (FNP) certification required.
• Master’s degree in Nursing or related field.
• Minimum 3 years of clinical experience as a Nurse Practitioner.
• Strong knowledge of Primary Care and Urgent Care practices.
• Excellent communication, clinical judgment, and interpersonal skills.
• Ability to work independently and thrive in a hybrid work environment.
• Experience in holistic and comprehensive patient care preferred.
