• Must be located in or near Monterey County
• Must have valid DL and proof of Auto Insurance
• Must be able to work remotely from home but also be willing to be out in the field

Housing Navigator Overview:

Cal-AIM Housing Navigators play a crucial role in delivering quality housing services to members authorized by Managed Care Plans (MCPs). They provide transitional housing navigation, deposits, and tenancy services, collaborating with local Continuum of Care (CoC) systems, MCPs, and Enhanced Care Management (ECM) providers. Reporting to the Cal-AIM Program Manager, Navigators must be experienced in housing and homelessness, knowledgeable in HUD regulations, and skilled in communication and organization.

Key Responsibilities:

• Develop outreach strategies for isolated consumers.
• Assist members with healthcare, education, employment, and housing resources.
• Increase awareness of self-help programs and recovery organizations.
• Conduct intake assessments to determine housing needs and complete HMIS CES Surveys.
• Help members develop and implement individualized housing plans.
• Identify and secure affordable housing, assist with applications, and conduct pre-inspections.
• Mediate landlord/tenant disputes, educate on rights, and prevent eviction.
• Maintain accurate documentation and housing charts per agency and HUD standards.
• Support benefits advocacy, including SSI eligibility and application assistance.
