(Only QUALIFIED Healthcare Professionals accepted) Licensed Clinical Social Worker (LCSW) - Licensed Clinical Social Worker- CADC (Whitefish, Montana) -Loan Repayment Eligible Location!

We are seeking a LCSW to join a well-respected FQHC to work on their behavioral health team. The FQHC provides an integrated service line of medical, dental and behavioral health care services to create a team based approach to healthcare. As a member of the integrated care team the Behavioral Health Professional has an opportunity to partner with other providers to encourage and help patients receive the behavioral support necessary to impact their overall health. The LCSW can work a traditional 8:00am-5:00pm schedule or a 4|10s schedule. The Provider would receive competitive compensation; $60,000-$70,000+ depending upon experience with full benefits including Medical, Dental and 401K with Matching contribution. The provider would also receive $25,000 paid directly to the candidates student loans on an annual basis (2 year commitment in exchange for $50,000 per year).
The Licensed Clinical Social Worker is responsible for providing brief intervention and consultation in real time to patients in a primary care setting; managing the assessment, treatment planning and episodes of brief goal focused therapy for established patients; providing crisis intervention as needed; monitoring mental health referrals for patients with higher behavioral health needs; coordinating care for complex patients with co-morbid mental and physical health conditions; functioning as a group visit team health educator; acting as a consultant and resource to members of the care team on skillful and effective interventions with patients with chronic illnesses and mental health diagnoses; assisting with staff training as needed; functioning as chronic care self-management support expert and trainer at clinic sites; meeting minimum documentation requirements for all services rendered in a timely manner; ensuring data collection as appropriate and required; assisting with implementation of an effective Quality Improvement Program; as well as, demonstrating knowledge of the principles of growth and development over the life span.
