Patient Care Manager, Home Health (RN)

Roswell, GA (on-site)

Full-Time, Permanent Position

HealthCare Recruiters International (HCRI) is partnered with a recognized national provider of home health and hospice services to recruit a Patient Care Manager (PCM, an RN, to supervise the day-to-day clinical and office operations of a home health agency.

PCM Qualifications and requirements:

• Registered Nurse (RN) License
• Experience in home health care
• Management or supervisory experience in home health, 2 yrs preferred
• Knowledge of accepted professional standards and practice, including Medicare Conditions of Participation and relevant regulations.
• Use of EMR systems.

PCM Responsibilities:

• PCM will supervise clinical and office operations and ensure compliance with regulations and serve as back-up support for other roles.
• PCM will coordinate referrals, develop and implement individualized care plans, and evaluate the quality of care provided.
• PCM works with all team members and physicians to coordinate patient care, including transfers and discharges.
• PCM ensures clinicians will have the required training, competency, and supervision.
• PCM may be involved in reviewing discharge assessments, OASIS data, and other relevant documentation.
• PCM will ensure schedules are updated, continuity of care for the patient episode is present, and that the clinical care team delivers quality care.
• PCM will monitor KPIs and manage workflows and will address patient concerns or complaints.
• PCM will facilitate case conference and be responsible for follow-up action related to case conference, stand-up, and stand-down meetings.
• PCM will be responsible for workday tasks related to employee supervision – PTO, Evaluations, Coaching Notes, etc.
