Job Description

Summary:

Advanced Management is seeking a Post-Acute Program Manager to support post-acute care operations and drive Medicare savings. This role works closely with the Post-Acute Case Manager RN to manage administrative functions, track performance metrics, and analyze post-acute care trends. The ideal candidate will have a strong background in healthcare administration and data analysis to help reduce hospital readmissions, optimize post-acute length of stay, and improve care coordination.

Responsibilities:

• Oversee daily administrative functions and support case management operations
• Collaborate with post-acute care providers, home health agencies, and internal teams
• Analyze and track post-acute utilization trends, readmission rates, and cost-saving initiatives
• Develop and implement strategies to improve care coordination and patient outcomes
• Ensure compliance with CMS regulations, Medicare policies, and ACO REACH guidelines
• Maintain accurate documentation and reports for compliance audits
• Foster relationships with post-acute partners, skilled nursing facilities, and rehabilitation centers

Qualifications:

• Education: Bachelor’s degree in healthcare administration, business administration, public health, or a related field (Master’s preferred)
• Experience: 2 or more years in healthcare administration, case management support, or post-acute care coordination. Experience with Medicare Advantage, ACO REACH, or value-based care models is a plus
• Skills: Strong data analysis abilities, Strong in healthcare reporting tools such as Excel, creating trackers, charts, pivot tables, KPIs, excellent communication skills, and a collaborative mindset
• Must live in Palm Beach County, FL

Starting Salary: $65,000+ (Commensurate with Experience)
