Job Description: Clinical Intake Manager
Summary and Objective
The clinical intake manager (CIM) is primarily responsible for facilitating timely admissions into CHS’s SNF, IRF, home health (HH), and ALF lines of business through a consistent presence in primary referral sources and conversations with patients and their caregivers. The CIM role, upon referral, is to interface with the appropriate hospital/clinic provider and case management personnel making the referral to collect and analyze critical patient information and determine the most appropriate post-acute level of care for admission to CHS. The incumbent is the face of CHS to potential referrers, patients, and the community. As a leader at CHS, this role is responsible for developing a strong culture and effective communication among the sales and intake team to achieve budgeted occupancy rates for the assigned CHS region/campus in conjunction with the supervising sales manager and associated EDs.
